Continuity depends on information carried between interactions. A model works from the conversation, instructions, and tools available to it. To continue earlier work, the relevant context has to be present again.
That explains an important part of returning to a conversation. It leaves the surrounding machinery mostly unnamed, and says too little about how a recovered record changes the next act.
